HEALTHY PREGNANCY & PARENTING
NURSE FAMILY PARTNERSHIP
Nurse-Family Partnership is a national recognized program that provides free nurse home visitation to low-income, first-time mothers. With the aid of a personal nurse, new mothers learn about the growth and development of their baby, get the support they need to become more self-sufficient, and gain the confidence needed to provide the best care for their child. Mothers also enjoy the benefits of community play dates, parent training, mentoring, and access to community resources.
HOW TO ENROLL
ELIGIBILITY REQUIREMENTS
- First time mom
- Less than 28 weeks pregnant
- Not enrolled in any other home-visitation program
- Classified as income eligible according to NC Poverty Guidelines

WHAT TO EXPECT FROM OUR NURSES
DURING PRENATAL VISITS
- Weekly visits beginning at 14-16 weeks gestation for the first month after enrollment, then bi-weekly until your baby is born
- Guidance on what to expect during pregnancy, delivery, and the first few months of your baby’s life
- Assistance with establishing personal goals and becoming more self-reliant
POST-PARTUM
- Weekly visits for the first six weeks
- Visits twice per month until your child reaches 20 months
- Monthly visits until your child reaches two years old
- Education about the growth and development of your baby
- Support as you continue working towards achieving your personal goals
